So, this just happened this week with Sleazee, where SWARTT's blood challenge went through as a regular challenge.

SWARTT SIXCLAW (358) (90-67-1)*              SERANIE (2130) (101-33-2)                    2              

I've had this happen to me at least twice, maybe three times.  The first time was a long time ago, when I first started playing.  Since this happens so rarely, I wonder if Keeper even knows it is a bug?  Anyone else have this happen to them?  When this bug happens, and you have a "you blood challenged your foe" trigger in place, it can be quite frustrating for it not to activate.
